ITEMS FOR REGISTRATION
The South
Country Central School District utilizes a
centralized registration process. All
registration takes place at the
Administration Building of the District,
located at
189 Dunton Ave, E. Patchogue, NY.
Registration is open
from
8:00 AM to 3:00 PM daily.
In order to
safeguard the health of your youngster,
place your child in the most appropriate
program, and to conform with New York State
Law and district policy, we need appropriate
information and records. All documents
provided will be held in confidence.
Listed below are the items necessary to
complete registration. Please note,
your child will not be enrolled until
the South Country Central School District
receives and verifies the required
documentation.

BIRTH
CERTIFICATE
A birth
certificate with a raised seal must be
provided for Pre-K and Kindergarten entrants
A photocopied birth certificate is
acceptable for entrants in grades 1-12
IMMUNIZATION
RECORDS
Certificate of
immunization signed by a physician.
Certificate of immunization signed by a
representative of an official health clinic.
School health record signed by official.
Physical examination form signed by a
physician.
PROOF
OF CUSTODY OR GUARDIANSHIP
(if
applicable)
Custodianship:
custodial and parental affidavits (original
or notarized copies).
Foster Parents: placement order from
Department of Social Services 2999.
Guardianship: guardianship document signed
by court officer (originals or notarized
copies) .

Kindergarten
Eligibility:
To be eligible
for Kindergarten, students must reside
within the geographic boundaries of our
school district and must turn five years of
age on or before December 1st of the school
year during which the child commences the
program.
Pre-K Eligibility:
To be eligible
for Pre-K, students must reside within the
geographic boundaries of our school district
and must be four (4) years old on or before
December 1 of the school year during which
the child commences the program.
